Hopelessly homely? Terminally ill? You may qualify for an Extreme Makeover. That's right, our favorite feel-good plastic surgery show is back.

Application details after the jump
 

1.) Print out and complete the Application Form.

2.) Include two recent photos (one full body and one close-up), at least 2"x2", with your name written on the back.

3.) Make a three minute videotape telling them why you want to be a candidate for Extreme Makeover.

4.) Mail the Application Form, photos, and videotape to the following address:

Will you ever offer this to persons outside the US. I live in Bermuda and in the process of divorcing. My self esteem is very low and would love for someone to give me an extreme makeover to start my life over. Unfortunately, I do not live in the US. Could I travel to the US and be eligible that way???? Any suggestions etc would really help.

Would you consider people living outside USA, i am living in Australia and i watch every episode of extreme makeover and i would love to have a makeover because i was born with a birthmark on my face and i would love to have it removed and be transformed into a beautiful person on the outside as i am a wonderful lady on the inside

hi,i am living in adelaide south australia,im a single mother of two young boys who would love to meet someone but lack lots of self confidence in my appearance and would like a boost so i could meet someone for me and a father for my boys as they dont have one.i was wondering if you take people from over here or if there was someone over here that does the same show as im getting desperate and older to meet people. Im 39 years old and the age is creeping up on me.
